MATERNITY HOME TAX CREDITS

70% Maternity Home Tax Credits are available to all donors that make a gift to Catholic Charities of Southern Missouri’s LifeHouse Crisis Maternity Home program. 

Important Tax Credit Information:
Tax Credit Usage: The tax credits you receive for a gift may be applied to your taxes for the year of the donation OR the following year. 
Deadline: The application must be submitted within 12 months of the donation date.
Tax Credit Limit: The amount of tax credits you can claim per year is capped at the amount of your state tax liability, with a maximum claim of $50,000 annually.

You will receive the MHTC application (see form pictured below) in the mail after you make your gift. If you do not have the form, click the “Download the MHTC Application” button below to access a printable version of the form. 

  1. Fill out the top portion of the form in blue ink (refer to the sample form below). 
  2. Provide your Social Security number(s) as required by the state. 
  3. Sign the “Taxpayer Signature” section at the bottom of page one. 
  4. Return the ORIGINAL FORM to:

  1. Fill out the form according to the instructions on the “Steps to Complete the Application” tab.
  2. Mail (or drop off) the completed form to:

  3. After signature by our CEO, we will mail the completed application to Jefferson City (typically mailed next day).
  4. You will receive a certificate acknowledging your tax credits within 45 days of receipt in Jefferson City.
